安全管(guan)理部部長武輝通報(bao)公司近期“三違”事件(jian)并向大會作七、八月份安全生產工作總結匯報(bao)。 七(qi)、八月份(fen)我公司發生硫(liu)氨2#飽和(he)器滿流槽閃爆(bao)、煉(lian)焦(jiao)熱調班員工(gong)(gong)(gong)(gong)瀝青灼燙、備(bei)(bei)煤機修工(gong)(gong)(gong)(gong)手指碰傷三(san)起(qi)工(gong)(gong)(gong)(gong)傷事故(gu),暴露出在安(an)全生產特(te)殊作(zuo)(zuo)(zuo)(zuo)(zuo)業中還存在監(jian)管(guan)(guan)(guan)(guan)(guan)(guan)不到位等(deng)安(an)全隱患,安(an)全生產管(guan)(guan)(guan)(guan)(guan)(guan)理(li)(li)工(gong)(gong)(gong)(gong)作(zuo)(zuo)(zuo)(zuo)(zuo)任重(zhong)道遠。七(qi)月份(fen)查處“三(san)違”行為258例,考(kao)核金(jin)(jin)額(e)33620元;八月份(fen)查處“三(san)違”行為241例,考(kao)核金(jin)(jin)額(e)15960元;這些“三(san)違”行為主要表現在私自解除聯(lian)鎖、違規停車(che)、辦理(li)(li)作(zuo)(zuo)(zuo)(zuo)(zuo)業票(piao)據(ju)不規范、承包商管(guan)(guan)(guan)(guan)(guan)(guan)理(li)(li)等(deng)方面。七(qi)、八月份(fen)安(an)全管(guan)(guan)(guan)(guan)(guan)(guan)理(li)(li)部完(wan)成各(ge)類隱患排查共計(ji)(ji)1862項,已整改(gai)1781項,整改(gai)率96%、專(zhuan)(zhuan)項檢(jian)查、特(te)殊作(zuo)(zuo)(zuo)(zuo)(zuo)業管(guan)(guan)(guan)(guan)(guan)(guan)理(li)(li)、特(te)種設(she)備(bei)(bei)管(guan)(guan)(guan)(guan)(guan)(guan)理(li)(li)、承包商管(guan)(guan)(guan)(guan)(guan)(guan)理(li)(li)等(deng)工(gong)(gong)(gong)(gong)作(zuo)(zuo)(zuo)(zuo)(zuo);推(tui)進了(le)消防安(an)全驗(yan)(yan)收(shou)工(gong)(gong)(gong)(gong)作(zuo)(zuo)(zuo)(zuo)(zuo),消防審(shen)圖單位出具審(shen)圖意見,督促溝(gou)通形(xing)成最終意見;組(zu)(zu)織(zhi)(zhi)(zhi)完(wan)成易制毒化學(xue)品硫(liu)酸(suan)(suan)、鹽酸(suan)(suan)備(bei)(bei)案(an)(an)登記(ji)工(gong)(gong)(gong)(gong)作(zuo)(zuo)(zuo)(zuo)(zuo);組(zu)(zu)織(zhi)(zhi)(zhi)、聯(lian)系檢(jian)驗(yan)(yan)機構對(dui)(dui)各(ge)區(qu)域(yu)空氣(qi)呼吸器18具氣(qi)瓶(ping)分批進行年(nian)度(du)檢(jian)驗(yan)(yan)、10臺(tai)(tai)壓(ya)力容器檢(jian)驗(yan)(yan);組(zu)(zu)織(zhi)(zhi)(zhi)公司1017人完(wan)成崗中職業健康體檢(jian),其中50歲(sui)以上246人,根據(ju)體檢(jian)報告(gao)完(wan)善一(yi)人一(yi)檔,同時(shi)對(dui)(dui)異常人員建立臺(tai)(tai)賬跟蹤(zong)管(guan)(guan)(guan)(guan)(guan)(guan)理(li)(li);組(zu)(zu)織(zhi)(zhi)(zhi)18名焊工(gong)(gong)(gong)(gong)、4名煤氣(qi)工(gong)(gong)(gong)(gong)、3名高壓(ya)電工(gong)(gong)(gong)(gong)及6名低壓(ya)電工(gong)(gong)(gong)(gong)、6名起(qi)重(zhong)設(she)備(bei)(bei)、15名制冷、16名高處作(zuo)(zuo)(zuo)(zuo)(zuo)業工(gong)(gong)(gong)(gong)外出培訓(xun);組(zu)(zu)織(zhi)(zhi)(zhi)各(ge)車(che)間(jian)按計(ji)(ji)劃及方案(an)(an)要求(qiu)開展應(ying)急演練(lian)工(gong)(gong)(gong)(gong)作(zuo)(zuo)(zuo)(zuo)(zuo),七(qi)、八月共計(ji)(ji)開展專(zhuan)(zhuan)項演練(lian)2次(化產車(che)間(jian)1次、干(gan)熄焦(jiao)車(che)間(jian)1次);現場(chang)處置(zhi)方案(an)(an)應(ying)急演練(lian)11次(化產車(che)間(jian)5次、備(bei)(bei)煤車(che)間(jian)1次,煤氣(qi)儲(chu)配站1次,煉(lian)焦(jiao)車(che)間(jian)2次、變電站1次、儀(yi)表車(che)間(jian)1次)。 下(xia)一(yi)步,安全(quan)管(guan)理部(bu)將圍繞四個重點(dian)開展工(gong)作:一(yi)是(shi)(shi)扎實(shi)推進安全(quan)風險(xian)(xian)(xian)辨(bian)識(shi)工(gong)作,充(chong)分辨(bian)識(shi)公司60個崗位存在的風險(xian)(xian)(xian)危(wei)害,制(zhi)(zhi)定(ding)管(guan)控(kong)清單(dan);二是(shi)(shi)通過培訓(xun)、考試等方法(fa)全(quan)面提(ti)(ti)升安全(quan)管(guan)理人員業務水(shui)平,提(ti)(ti)高對安全(quan)管(guan)理工(gong)作的認知;三是(shi)(shi)認真(zhen)學習(xi)《化(hua)(hua)工(gong)過程安全(quan)管(guan)理導(dao)則》,加強(qiang)過程管(guan)控(kong),修(xiu)訂全(quan)員安全(quan)生產責任(ren)制(zhi)(zhi),監督(du)各級(ji)應急預(yu)案的演練,細化(hua)(hua)承包商管(guan)理,強(qiang)化(hua)(hua)特殊作業風險(xian)(xian)(xian)管(guan)控(kong);四是(shi)(shi)依(yi)托智(zhi)能化(hua)(hua)平臺,提(ti)(ti)升整體監管(guan)水(shui)平。 公司電氣二車間主任(ren)雷小兵(bing)和儀表車間主任(ren)郭(guo)艷(yan)紅分別(bie)向大會分享學(xue)習了典型的電氣和儀表事(shi)故(gu)(gu)案例。通過對(dui)事(shi)故(gu)(gu)案例的學(xue)習和事(shi)故(gu)(gu)原因的認真剖析,教育大家吸取事(shi)故(gu)(gu)案例教訓,舉(ju)一反(fan)三,防微杜漸(jian)。在日(ri)常(chang)生產過程中(zhong),牢(lao)固(gu)樹立“隱患就是事(shi)故(gu)(gu)”“隱患不除永無寧日(ri)”的安(an)(an)全(quan)理念,扎實做好安(an)(an)全(quan)生產預防管(guan)理工作,堅(jian)決杜絕各類(lei)安(an)(an)全(quan)事(shi)故(gu)(gu)發生。 公司副總經理劉(liu)銘(ming)亮針(zhen)對(dui)承(cheng)包商施(shi)工(gong)情(qing)況提(ti)出了九個(ge)突出問題:1、組織管理機構(gou)不(bu)(bu)健(jian)全;2、特殊作(zuo)業(ye)人(ren)員(yuan)作(zuo)業(ye)證(zheng)不(bu)(bu)齊全,明確要求焊工(gong)必須要有(you)高處作(zuo)業(ye)證(zheng);3、管理制度不(bu)(bu)健(jian)全、不(bu)(bu)完(wan)善(shan),部分(fen)(fen)制度不(bu)(bu)符合(he)(he)實際(ji)情(qing)況;4、流動施(shi)工(gong)人(ren)員(yuan)無(wu)備案(an);5、施(shi)工(gong)方案(an)不(bu)(bu)完(wan)善(shan),部分(fen)(fen)施(shi)工(gong)方案(an)不(bu)(bu)貼合(he)(he)實際(ji)且作(zuo)業(ye)票證(zheng)無(wu)相關負責人(ren)簽(qian)字(zi);6、承(cheng)包商使用的部分(fen)(fen)工(gong)作(zuo)器具(ju)(ju)超(chao)出使用年限標準,存在(zai)安全隱患;7、在(zai)消防方面存在(zai)管理缺陷,沒有(you)配備滅(mie)火器具(ju)(ju);8、未給施(shi)工(gong)人(ren)員(yuan)繳納(na)工(gong)傷保險(xian),僅投保意(yi)外險(xian);9、對(dui)部分(fen)(fen)崗(gang)位(wei)職責不(bu)(bu)清(qing),現場監護不(bu)(bu)到(dao)位(wei)。 同時(shi),劉(liu)總(zong)針(zhen)對公司安(an)全(quan)管理工作(zuo)又(you)提出七(qi)方(fang)面(mian)的問題:1、作(zuo)業過程中風險辨識不(bu)(bu)(bu)到位;2、廠區(qu)安(an)保管理不(bu)(bu)(bu)嚴(yan),有(you)無關人員(yuan)隨意(yi)進出現(xian)象;3、焦廠粉(fen)塵較(jiao)大,棚內停放車(che)輛數(shu)量(liang)較(jiao)多;4、提鹽車(che)間存在“跑、冒、滴(di)、漏(lou)”現(xian)象;5、檢(jian)維修時(shi),作(zuo)業現(xian)場無關人員(yuan)聚集;6、部(bu)分(fen)探頭(tou)報警(jing)時(shi)處置方(fang)法不(bu)(bu)(bu)當,速(su)度不(bu)(bu)(bu)快;7、安(an)全(quan)員(yuan)綜合素質有(you)待提升。 同時,結(jie)合上(shang)述問(wen)題整改及公(gong)司(si)下(xia)(xia)一(yi)步安全(quan)(quan)(quan)中心工(gong)作(zuo)進(jin)行(xing)(xing)了安排部署(shu):一(yi)、要認真學習和貫徹公(gong)司(si)下(xia)(xia)發(fa)的(de)《關于全(quan)(quan)(quan)面開展安全(quan)(quan)(quan)風險辨(bian)識評(ping)估工(gong)作(zuo)》文件精神,嚴格落實并(bing)進(jin)行(xing)(xing)公(gong)示(shi);二、重(zhong)大(da)危(wei)險源評(ping)估工(gong)作(zuo),安全(quan)(quan)(quan)部要備案(an),及時溝通解決涉及的(de)問(wen)題;三、進(jin)一(yi)步完善電動(dong)車(che)停放等管理制度(du);四(si)、安全(quan)(quan)(quan)部組織(zhi)對各車(che)間進(jin)行(xing)(xing)事故分類檢查;五、對體檢異常員(yuan)工(gong)進(jin)行(xing)(xing)日(ri)常監管,適(shi)當進(jin)行(xing)(xing)崗(gang)位(wei)調整;六、安全(quan)(quan)(quan)部牽頭組織(zhi)學習《化工(gong)過程安全(quan)(quan)(quan)管理導則》,并(bing)出臺相關制度(du)。 會上,劉總還組織集體學習了《化(hua)工過程安(an)全管(guan)理導則》部(bu)分內(nei)容。 公(gong)司總經理(li)(li)張軍圍繞公(gong)司今后(hou)安(an)(an)全(quan)生產管理(li)(li)工(gong)(gong)作,提出(chu)五點要(yao)(yao)求:一是(shi)(shi)要(yao)(yao)強化(hua)生產現場管理(li)(li),尤(you)其是(shi)(shi)承包商安(an)(an)全(quan)施工(gong)(gong)管理(li)(li),做好(hao)查漏(lou)補缺(que)工(gong)(gong)作,列出(chu)問題清單(dan),確(que)保閉環管理(li)(li);二是(shi)(shi)要(yao)(yao)強化(hua)領導(dao)干部安(an)(an)全(quan)責任意(yi)識(shi),杜絕麻痹思想(xiang),抓好(hao)隱(yin)患的整改工(gong)(gong)作;三(san)是(shi)(shi)要(yao)(yao)增強所有干部員工(gong)(gong)安(an)(an)全(quan)風(feng)(feng)險辨識(shi)意(yi)識(shi),樹立“管生產必須管安(an)(an)全(quan),管安(an)(an)全(quan)必須管風(feng)(feng)險”的安(an)(an)全(quan)理(li)(li)念(nian),全(quan)流程、全(quan)崗(gang)位,不留死角(jiao)做好(hao)風(feng)(feng)險辨識(shi);四(si)是(shi)(shi)要(yao)(yao)嚴(yan)格(ge)按(an)照(zhao)檢(jian)修(xiu)作業方案(an)進行(xing)檢(jian)修(xiu),堅(jian)持方案(an)論證原則,每一項檢(jian)修(xiu)工(gong)(gong)作,必須經過安(an)(an)全(quan)技術論證后(hou)方可執行(xing)。按(an)照(zhao)安(an)(an)排部署、檢(jian)查落實、問題整改、總結(jie)提升的工(gong)(gong)作流程,形成(cheng)安(an)(an)全(quan)檢(jian)修(xiu)閉環管理(li)(li);五是(shi)(shi)要(yao)(yao)強化(hua)“上(shang)標(biao)(biao)準(zhun)崗(gang),干標(biao)(biao)準(zhun)活”,嚴(yan)格(ge)按(an)規程,規范(fan)作業。提高安(an)(an)全(quan)確(que)認能力(li)、規范(fan)能力(li)和工(gong)(gong)作效率。
